Responsibilities Job Summary / PurposeThe RN-Navigator navigates cancer patients through the continuum of cancer care providing education, interpretation, emotional support, support group opportunities, etc.
for oncology patients and their care givers.
The nurse serves as the primary point of contact coordinating and collaborating with multiple physician specialties for care planning and services across the continuum of care effectively utilizing resources and multidisciplinary teams to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.
Essential Key Job Responsibilities Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the cancer patient at the completion of their treatment in all aspects of the continuity of care.
Tracks patient progress throughout the course of diagnosis, surgery, treatment to completion.
Systematically and continually performs the functions of assessing, planning, implementing and evaluating the care according to NCCN guidelines, COC standards, NAPBC requirements and quotas, CHI Saint Joseph Health standards, and ONS standards of practice.
Facilitates the coordination of patient care post treatment to ensure there are no follow-up barriers, appropriate referrals are made, and the patient understands what resources are available to them (i.
e.
support groups, financial navigation, follow-up appointments, etc.
) Prepares survivorship care plans for the applicable patients and reviews and edits the care plan templates as needed.
Identify patients that qualify for survivorship program visits, contact the patient, and coordinate the scheduling of the survivorship visit.
Initiates and documents patient teaching including family and significant others based on assessment of needs, including responding to patient request for information regarding the disease process, expected side effects of treatment and community resources.
Documents the above appropriately as well as communicating teaching completed to the patients other health care providers as appropriate.
Active participant on the multidisciplinary care team and conferences, providing specific patient care information pertinent to the patient, follows up on patient care recommendations following the multidisciplinary conferences, and when necessary, facilitates the multidisciplinary conference.
Facilitate/organize patient support groups as appropriate.
Serves as a resource for community educational events such as health fairs, screenings, support groups and utilizes the resources of CHI Saint Joseph Health organization.
Other duties as assigned by management.
Qualifications Minimum QualificationsRequired Education (for CHI Leadership Job Levels from Supervisor through President) Does Not ApplyRequired Education for Staff Job LevelsBachelors of Science in Nursing required.
Required Licensure and CertificationsMust have a current RN license to practice in the state of Kentucky.
OCN Certification required.
Required Minimum Knowledge, Skills and AbilitiesMinimum of 5 years' experience in inpatient or outpatient oncology, surgical services or related.
PREFERRED QualificationsExperience/knowledge in community health setting or services desirable.
Recommended Skills Communication Community Health Finance Health Care Multidisciplinary Approach Nursing Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
